Eligibility List

In order to get your child in the eligibility list you must submit the following:

• Eligibility List COMPLETELY filled out - Forms that are not properly filled out won't be processed.
• Proof of Income - One month's worth of paystubs, or tax returns in paid cash.

At this time, please DO NOT use the "submit" button found the form. Print out the filled-out form and submit it to us via email, mail, or hand-deliver.
